In order to honor our volunteer team for their years of dedication and hard work, we will be conducting an online format for sponsorship and all gifts will be mailed directly to the Foster Home and all requested items will need to be from Amazon. We will return to our traditional sponsorship program in 2027. Please follow all of the instructions on the form.

FILE UPLOADS:

  • You will be asked to upload your custody letter, driver’s license and a photo of your foster child on this form. Failure to do so will result in your application being cancelled.

  • Photos will need to have only that child in the image (preferably smiling) with their eyes covered with heart, sunglasses or star emojis. This will give your sponsor a deeper connection to your foster during this experience since they will not be physically shopping for your child this year.

IF YOU DO NOT FILL OUT THE FORM, FOLLOWING ALL INSTRUCTIONS, WE WILL CANCEL YOUR REQUEST AND ASK YOU TO COMPLETE IT AGAIN.

Please complete one form for each foster child, even if they are a part of a set of twins or triplets.
