The child that I applied to be sponsored is currently a foster child recognized by the state, is legally in my custody and if that child leaves my custody between now and the Pick Up Event on December 18, 2025 at Madison Street Baptist Church of Starke in Starke, Florida. I will contact Christmas for Fosters and let them know.

I understand that foster children will be matched on a first come, first serve basis.

I understand that I am responsible for picking up all of the gifts at the Pick Up Event on December 18, 2025 at Madison Street Baptist Church of Starke in Starke, Florida (other appointments are not available - ZERO EXCEPTIONS).

I understand that I have to provide a hard copy of the foster child's custody letter (digital copies will not be accepted) and government form of identification to be able to pick up gifts.

I understand that only those who are listed on the custody letter as foster parents/guardians are able to pick up the gifts - no exceptions. Please do not ask your case managers, licensing specialists or guardian ad litems to pick up the gifts.

I understand that if gifts are not picked up at the Pick Up Event that I forfeit the gifts to be give to another child or to be distributed to a local foster closet or high-needs family.

I understand that this is on behalf of Give Joy Collective, local foster mamas and foster care advocates founded by Sarah Weaver.

I understand that the only day, time and location that I can pick up is on December 18, 2025 at Madison Street Baptist Church in Starke, Florida

I understand that if I am found to be deceptive about any information on this form that my household will be banned from participating in any program affiliated with Give Joy Collective.
